Summary :
- Provides support to the patients, providers and families of patients who are under the care in clinics; provides timely information to patients and families of patients who are undergoing treatment or medical procedures;
- helps to streamline communication between clinics, OR providers, PACU, ICU and patients’ families; services patients at Roswell Park;
- ensures that continuous quality care is received by all patients inside and outside of the clinic setting; monitors and assists in the throughput of patients in an ambulatory center;
- assists in ensuring clinic efficiency by supporting providers during clinic; assists with the connection of patients from community and clinical referrals for assessment and treatment;
- assists providers and patients in accessing needed resources and services to allow them to move through Roswell Park quickly and efficiently;
- coordinates with internal services and outreach programs with local and regional health centers, to facilitate timely cancer care and provide encouragement of adequate patient adherence;
serves as both an internal and external resource for hospitals, screening facilities, county health units, state health department, patients, physicians, lay public, volunteers, and graduate students for information regarding Roswell Park.
Salary range for this position is $49,363 - $63,169 annually which includes a comprehensive benefit package.
Qualifications :
Required Education and Experience
1. Bachelor’s degree in a health related field and the equivalent of one (1) year of full-time experience in community, patient or customer-oriented support; or
2. Associate’s degree in a health related field and the equivalent of three (3) years of full-time experience in community, patient or customer-oriented support.
3. High School Diploma or High School Equivalency Diploma and the equivalent of five (5) years of full-time experience in community, patient or customer-oriented support.
