Position Title : Home Health Manager

Expected Duration : 4 Months

Job Type : Contract with Possible extension

Shift : 7am - 5pm

Job Description :

The Manager Home Health is responsible for the delivery of effective and efficient care within assigned areas / disciplines and directs, organizes and supervises the service provided by professional staff.

This position supervises staff, prepares and forecasts budgets and strategic plans, participates in quality assurance programs and ensures regulatory compliance.

Essential Functions

Reviewsprogramforqualityofcare,accuracy,appropriateutilizationofservicesandcompliance with reimbursement criteria.

PerformanceManagement

Supervisesdirectreportsandmanagestheperformanceofindividualsthroughongoingcoaching, feedback, and development to motivate, engage and drive a high performing team.

PerformanceManagement

Monitorstheoperationsofaunitandprovidesinputtotheperformanceofindividualsthrough feedback and recommendations.

PeopleManagementActivities

Makesdecisionsfordirectreportsandperformspeoplemanagementactivitiessuchas performance evaluations, disciplinary actions, staff planning, and interviews.

BudgetandPolicies

Participatesinthedevelopment,implementation,andmanagementofabudget,ensuringcost control and expenses are in alignment with the budget.

PatientCareImprovements

Essential Functions

TIME

Compilesandanalyzesclinicaloutcomesdataandimplementingimprovementplanstoensure high-quality patient care in accordance with established standards and regulations.

Compliance

EnsurescompliancewithallapplicableregulatoryagenciesandinternalpoliciesincludingFederal, State, payor, and CHAP (Community Health Accreditation Program).

ComplaintsManagement

Investigates,evaluates,andresolvesconcernsofthoseweareprivilegedtoserveandreportsto the next level of management.

PatientSatisfactiondata

Monitorspatientsatisfactiondataandidentifies opportunitiestoenhancethepatientexperience.

PatientCareServicesOversight

Provides oversight of all patient care services. Oversight includes : coordinating patient care, coordinatingreferrals,ensuringthepatientneedsarecontinuallyassessed,andensuringthe development, implementation, and updates of the individualized plan of care.
