RN Team Leader Day Shift

Job Summary :

Supervises, evaluates, and coordinates the activities of the Care Partner & Coming Home Team members.

What You Should Know About the Team Leader Care Coordination Center Role :

• This is a full-time exempt position, M-F 8am-5pm, no weekends, no on call
• The coverage territory will be for Miami County and surrounding areas
• This position will included driving to facilities / team centers as needed.
• We provide superior care and superior services to patients at their end of life journey. Only those who have a heart for hospice will succeed.

Essential Duties :

Supervises, evaluates, and coordinates assigned members of the interdisciplinary team; assure continuity of care from admission to discharge or transfer to bereavement;

serve as patient advocate and coordinator for other providers who are involved in the patient’s care.

Qualifications :

• Associate of Science in Nursing (ASN); Bachelor of Science in Nursing (BSN) preferred
• Active Registered Nurse license in the State of Ohio without any board actions.
• Minimum two years nursing experience and two years management experience preferred.
• Certification in Hospice & Palliative Medicine (CHPN) preferred; required within two years of employment. (Eligible upon completion of 500 hours in last 12 months of hospice / palliative experience.) CPR Certified
• Computer skills sufficient to properly document services and care
• Travel may be required during daytime, nighttime or inclement weather.
• Valid Driver's License with Safe Driving Record
• State Minimum Automobile Insurance Coverage
