We are looking for Registered Nurses in Nassau County to create Care Plans in the home setting and complete required follow up visits.
Qualifications / Education :
- 3 years of RN Experience
- Experience in completing field evaluations in a home setting, start of care assessments, follow up visits after hospital.
- Resume with experience in opening home health cases, follow up visits.
- AlayaCare experience preferred.
Responsibilities :
- Utilize our EMR AlayaCare for nursing notes
- Perform physical exams and health histories before making critical decisions.
- Provide health promotion, counseling, and education.
- Administer medications and other personalized interventions.
- Coordinate care, in collaboration with a wide array of health care professionals.
- Document nursing notes regarding patient care daily.
- Must be able to lift 50lbs.
- Must be able to stand / work for extended periods
- Additional responsibilities as required.
