RN Supervisor Job Description
Job Summary :
Responsible for day-to-day oversight of operations and supervision of the Agency service delivery. Manages direct client services, managing the weekly schedules of client service employees, taking the referral intakes as needed, conducting client record reviews and participating in the QA activities.
Organizational Relationship : Reports directly to the Agency Manager Risk Of Occupational Exposure To Blood Borne Pathogens : C : no exposure Qualifications :
Graduate of an approved professional nursing program (RN)
Currently, licensed in good standing in WI as an RN under s. 441.06 Wis. Stats. Training and experience in the provision of PCS or in a related program.
At least, of one (1) year of supervisory or administrative experience in personal care services or in a related program.
Must have a criminal background check.
Must have current CPR certification.
Responsibilities :
- Evaluate the need for services and make referrals to other services as appropriate.
- Secure written orders form client’s physician, which are renewed once every 3 months unless the MD specifies that orders covering a period of time up to one year are appropriate or when the client’s needs change, whichever occurs first.
- Develop a plan of care for each client, giving full consideration to the client’s preferences for service arrangements and choice of staff, interpret the plan to the PCW, include a copy of the plan in the client’s health record and review the plan at least every 60 days and update if necessary.
- Promptly notify a client’s physician or other appropriate medical personnel and legal representative, if any, of any significant changes observed or reported in the client’s condition.
- Develop an appropriate time and service reporting mechanisms for staff and instruct the workers on their use.
- Give the staff written instructions about the services to be performed and demonstrate to the staff how to perform the services.
- Evaluate the competency of the staff member to perform the services.
- Documents telephone / in-office conferences with clients / family, staff, and others.
- Assists with interviews / hiring as needed.
- Establishes mutual goal setting and achievement standards.
- Maintains high visibility and availability while in the office.
- Works to develop a positive rapport / relationship with all staff.
- Performs tasks as delegated by Agency Manager.
- Shares on-call responsibility as required.
Functional Abilities :
- Must be able to hear and effectively communicate in English.
- Must be able to read 12 point or larger type.
- Must be able to travel to prospective clients' place of residence.
