RVCC is looking to hire a BSAS Case Manager!

Overview of Job Responsibilities :

To obtain required documentation from clients. Complete initial assessment and individualized service plan (ISP) in Electronic Health Record.

To provide information and referral services for consumer services based on assessed client needs.

To support individuals and their families in completing applications or various paperwork related to life management; i.

e.; medical coverage, housing, disability, etc.; when necessary and in a timely manner.

To provide or arrange for the provision of appropriate support services to assist program participants in maintaining independent living, such as budgeting, accessing parenting skills training, or any other skills' guidance, etc.; as appropriate

To provide outreach services to assist program participants in using and accessing needed community and medical services, including arranging or providing transportation.

To provide or ensure that English / Spanish written and / or verbal interpretation for Spanish-speaking clients of all ages and their families / significant others is provided when necessary.

Arrange interpretation for other languages as needed

Job Requirements :

Education Level - CHW Certification preferred, Bachelor’s Degree with a concentration in human services preferred. CPR Certification preferred and offered by RVCC.

Years of Experience - 1-year experience working with substance-use populations preferred

Skills / Abilities -. Bilingual (Spanish / English) ability strongly preferred

Licensing - None
