Position Summary :
As the Addictions Coordinator, Psychiatric Social Worker with Johns Hopkins Howard County Medical Center you follow the clinical care journey for patients.
You collaborate closely with the Nurse Manager and Clinical Director, driving the seamless implementation of patient care plans from admission to discharge.
This role champions innovation to optimize patient outcomes and cost-efficiency, demanding persuasive communication skills to influence and garner support for transformative ideas, both within and beyond their immediate team.
Responsibilities :
As a Psychiatric Social Worker, the responsibilities encompass comprehensive psychosocial assessments of patients and families, delving into emotional, social, and environmental factors affecting mental health.
The aim is to identify strengths, coping strategies, and areas necessitating intervention. Collaborating closely with the interdisciplinary team, individualized treatment plans are crafted, aligning interventions with the unique mental health needs and goals of each patient.
This role involves providing evidence-based individual and family counseling to address emotional and behavioral challenges.
Immediate crisis intervention is offered to those facing acute emotional distress or psychiatric emergencies. Furthermore, patient and family education is paramount, empowering them with knowledge about mental health conditions, treatment options, and available community resources to bolster coping skills and support recovery.
Effective discharge planning ensures a seamless transition to appropriate levels of care or community resources upon discharge.
Advocacy efforts extend to championing the rights and needs of patients and families both within the healthcare system and the broader community, facilitating access to vital services and support.
Active participation in interdisciplinary team meetings, case conferences, and treatment planning sessions ensures a holistic approach to patient care.
Rigorous and timely clinical documentation, including progress notes, treatment plans, and discharge summaries, adheres to hospital and legal guidelines.
Lastly, a commitment to continuous learning drives the professional development of this role, keeping them abreast of the latest advancements in pediatric mental health, evidence-based practices, and treatment modalities through ongoing training and development activities.

EDUCATION & EXPERIENCE :
Graduate of an accredited college / university.
MSW required.
A minimum of one (1) year clinical experience in substance abuse and psychiatric illness.
Experience in both individual and group therapy.
LICENSES & CERTIFICATIONS :
Current Maryland License as a Licensed Clinical Social Worker Certified (LCSW-C).
Current CPR certification.
