Supervises, manages, and provides oversight of pharmacy resident(s). Participates in research within the area of specialty;

instructs and trains pharmacy residents and students. Provides a high level of direct patient care pharmacy services for an assigned, specialized patient population to assure adequate patient care according to professional standards and practices.

  • Advises / consults with patients and their families and the medical and nursing staff. Participates in research and drug use and outcome management programs within specialty area;
  • responsible for publishing data or giving presentations at local, regional or national meetings; instructs and trains pharmacy residents and students;

assists in overseeing and directing the clinical services provided by pharmacists. Acts as a mentor to clinical pharmacists and other clinical specialists.

Coordinates Residency ProgramProvides oversight of pharmacy residents to ensure optimal pharmacy servicesEnsures compliance with the residency learning system set forth by American Society of Health-System Pharmacy (ASHP)Maintains ASHP accreditation of the residency programActively participates in the education mission of the department through the training of pharmacy students and residents.

Clinical Pharmacy ServicesParticipates in direct patient care with a medical and / or surgical team.Assists with therapeutic decisions such as drug product selection, drug dosing, and therapeutic drug monitoring.

Develops patient care plans in concert with the medical team.Recommends therapy or alternative approaches based on interpretation of relevant medical literature and clinical experience.

Makes verbal or written recommendations in a clear, succinct and appropriate manner.Considers age, weight and organ function (e.

g. renal, hepatic) when evaluating patients and patient’s therapies when making recommendations.Interviews patients and conducts physical assessments to determine medical history, previous medication condition and response to therapy.

Interprets laboratory and other patient-specific data to develop treatment plans and to monitor response to therapy.Applies pharmacokinetic principles in order to achieve efficacious, safe and economical pharmacotherapeutic regimens.

Responds to therapeutic queries posed by physicians and other healthcare providers.Identifies and corrects drug-related problems such as complications resulting from drug therapy (ineffective therapy, allergic reactions and adverse drug reactions).

Establishes procedures for detecting significant drug-drug, drug-laboratory and drug-food interactions. Make recommendations to avoid such interactions.

Performs basic cardiac life support and participates in drug therapy management during medical emergencies.Ensures a continuum of care by cooperating with other health professionals and outside health organizations.

Counsels patients and / or caregiver as to the proper administration, potential adverse reactions, storage of medications, etc.

Drug Use, Outcome and Formulary ManagementEnsures and enforces compliance with established clinical protocols and guidelines with the multidisciplinary care providers.

Identifies and establishes programs that improve drug therapy outcomes (efficacy, safety, cost-effectiveness) and overall quality of patient care.

Implements and monitors drug use management programs.Participates in the Medication Use Evaluation Program by designing drug use criteria, collecting patient information, providing accurate analysis and reporting data.

Participates in clinical pathway development and other related managed care initiatives such as disease management programs.

Participates in the formulary management process including development of drug monographs, treatment protocols, drug use policy, algorithms and guidelines within the purview of the P&T Committee.

Documents pharmaceutical care interventions conscientiously as part of the departmental documentation program. Staff Development, Education and TrainingProvides in-service education to physicians, nurses and other healthcare providers.

Expected to obtain clinical faculty appointment at the School of Pharmacy or School of Medicine.Serves as a preceptor for PharmD students, residents and staff.

May service as Program Director of a Residency Program.Attends hospital, medical and pharmacy department conferences and meetings.

Actively participates as a presenter in departmental staff development programs.Orients and trains new personnel in assigned areas.

Scholarly ActivitiesReads literature and attends continuing education sessions to stay current, expand knowledge and maintain clinical acumen.

Formulates and conducts departmental projects aimed at addressing Medical Center and departmental advancements in clinical and operational practice.

Participates in process redesign.Disseminates original research through publication in peer review literature and / or presentation of written or delivered reviews on current topics at local, regional or national meetings.

Minimum of one publication / speaking engagement a year.Conducts and / or participates in research. Ability to identify research issues, opportunities or problems for investigation.

Operational ResponsibilitiesProvides front-line management of service areas such as unit-based practice models, pharmacy satellites or ambulatory pharmacy settings.

Oversees, directs and participates in order entry, preparation, dispensing and distribution of medication in compliance with the operational policies and procedures.

Oversees and directs the pharmaceutical care activities of pharmacy technicians. LeadershipDisplays conduct and behaviors that model the mission, vision and values of the pharmacy department.

Maintains and complies with established department, state, federal and local laws, regulations, policies, procedures and generally accepted pharmacy practice.

Participates in Medical Center committees as directed.Models sound organizational and teamwork skills to accomplish departmental and Medical Center goals and objectives.

Enhances Customer RelationsInteracts with internal and external customers of the Medical Center in a respectful, courteous and tactful manner to foster the delivery of quality patient and employee services.

Evaluates customer’s needs and thereby develops systems and services to improve relations and satisfaction.Performs other duties as assigned.
