Summary
The Program of All-Inclusive Care for the Elderly (PACE) is a community-based program that provides coordinated medical and social services to eligible older adults who want to continue living in their own home despite chronic care needs.
The Enrollment Outreach Specialist under direct oversight of the Program Director, is an integral member of the PACE at Hudson Headwaters (PHH) team.
The Enrollment Outreach Specialist will lead Participant enrollment efforts to reach monthly enrollment goals, build referral sources, and maintain compliance with state and federal regulations.
The Enrollment Outreach Specialist will work to promote and maximize awareness of PACE within the community.
Essential Duties and Responsibilities
ENROLLMENT
- Lead team to meet monthly enrollment goals.
- Coordinate all aspects of the intake and enrollment process, in compliance with CMS PACE and NYS managed long term care (MLTC) guidelines.
- Thoroughly understand PHH’s eligibility criteria and the financial qualification process for Medicaid.
- Assist prospective Participants and families / caregivers with applications and documentation needed to facilitate enrollment and financial eligibility.
- Grow enrollment pipeline by identifying potential referral sources and executing engagement strategies to drive referrals.
- Directly supervise Enrollment Specialist(s).
- Maintain a database of qualified prospects.
- Conduct tours of the PACE facility and give individual presentations about the benefits of PACE.
- Work closely with Director of Operations & Team Services and Clinical Leader to effectively coordinate and administer level of care assessments, financial eligibility, home visits, and seamless handoff to Interdisciplinary Team (IDT).
- Maintain a strong working relationship with local and state regulatory agencies to ensure compliance with NYSDOH / MLTC and CMS guidelines and policies.
- Prepare, review, and update required data and reports.
- Comply with all marketing guidelines related to the PACE program and managed care as set forth by federal, state and agency guidelines and prohibited marketing practices.
- Develop and maintain relationships with internal and external stakeholders such as providers, long-term care facilities, geriatric social workers, care coordinators and related community organizations.
COMMUNITY OUTREACH
- Plan, organize and conduct educational programming within the community to raise awareness about the PACE program.
- Attend community networking events and develop strong working relationships with community partners.
- Give presentations to community partners and distribute marketing materials as needed.
Qualifications
- Bachelor’s degree from an accredited college and / or institution in health care administration, health-related or human services field or business administration / management, or licensed practical nursing
- 2+ years of career experience with marketing health and / or human support services, preferably involving services having enrollment and intake provisions with eligibility standards and requirements
- Have 1 year of experience working with a frail or elderly population or be willing to receive appropriate training upon hire
- Current BLS certification or willing to complete certification
- Knowledge of and / or experience with NYS Managed Long Term Care, Medicaid, Medicare, and senior care related programs strongly preferred
- Must possess strong customer service skills and build Participant-first culture within the PACE program
- Comfortable with monthly goals and productivity metrics
- Knowledge of systems and procedures to efficiently track and trend all enrollment activities
- Proficient experience with Office 365 (Word, Excel and Outlook)
- Must thrive in team environment, possess excellent organizational skills, and be able to effectively handle difficult interpersonal situations
- Excellent verbal and written communication skills
- Excellent time-management skills and ability to prioritize deadlines
- Must be able to work independently as well as collaborate and communicate effectively with colleagues, supervisors, service delivery partners, other health care professionals to build and maintain effective dynamic professional team relationships
The pay range for this position is $58,500-$69,700 annually and will be determined based on skills and experience.
