- Team-based collaborative community outpatient practice with a team of Family physicians, NPs, RNs, Pediatricians, OB-GYNs, Dentists, specialists (optometrist, podiatrist, dermatologist), LCSWs, mental health professionals, case managers, health educators, registered dietitians, LVNs.
- Strongly prefer bilingual in English / Spanish to deliver care in Spanish and English.
- Committed to providing high-quality care to underserved patients
- Primary care and chronic disease management comprises the bulk of care.
- Responsible for assessment, diagnosis, and management of healthcare needs, utilizing standardized and evidenced-based procedures.

Details :
- Full-time : 4 or 5 days / week
- Monday - Friday, 8AM 5PM
- It does require working one Saturday / month for which a weekday is flexed.
- Outpatient
