Today, there are 1.5 million Chicagoans in Sinai's service area. The Social Services Social Worker will be working in an Inpatient setting within Mount Sinai Hospital.
Position Purpose :
Medical social work- discharge planning
The Social Services Worker assists patients and their families with personal and environmental difficulties, which predispose illness or interfere with obtaining maximum benefit from medical care.
Collaborates with the nurse case manager and the multidisciplinary team to ensure timely progress through the system and transitions of care.
Performs core functions of assessment, advocacy, education and information, referral and linkage, coordination, and counseling and crisis intervention for assigned patients.
Qualifications :
Master's degree in Social Work (MSW) required.
Social services experience in a health care setting required.
Licensure by the State of Illinois as a Social Worker (LSW) or Clinical Social Worker (LCSW) required.
Provides assessment and discharge planning for medical and behavioral health patients.
