AmeriCorps Community Health Resources Navigator

Job Title : AmeriCorps Community Health Resources Navigator

Time Requirement : 35 - 40 hours / week, Monday - Friday

Location : Stevens Point

Summary of Position :

This is a part-time, grant-funded position through the Marshfield Clinic AMERICORPS Program. The purpose of this position is to advance access to community health resources, including access to health care, by serving as a navigator to clients of Portage County Health and Human Services.

The community health resources (CHR) navigator serves as an intermediary between public health or social services, government systems and a community.

The CHR navigator have a trusting relationship with the people they serve. Creating and maintaining contact is an essential part of their role.

They can facilitate their clients' access to quality services and educate them on health matters. The position may work in both urban or rural environments and may collaborate with communities that have limited resources to help find appropriate care resources.

For example, being able to explain the advantage of government programs to eligible residents in an underprivileged community and help them enroll.

This position transitions between social and community health work. Both positions involve building relationships with the populations and providing help.

Member Job Duties :

• Educating individuals on health issues and teaching them strategies to improve their well-being.
• Providing educational presentations in community settings and individually.
• Helping people learn how to find and access resources like quality care and health information, health insurance, housing or food.
• Putting communities in contact with health care or social services.
• Improving access to health screenings and referrals.
• Helping individuals complete applications for insurance or health services.
• Facilitating communication during interactions with health care or social service systems.
• Providing interpretation and translating services as needed.
• Coordinate transportation to access medical appointments.
• Increase access to and linkages with clinical care.
• Travel within Portage County and connect with the local people, visit homes, speak to groups, tour health care facilities and share information.
• Collecting, sharing, and reporting of data.

Minimum Qualifications :

• Bachelor’s degree from an accredited college in a related field or equivalent experience is preferred
• One or more years of conducting health coverage outreach, eligibility, and enrollment is preferred.
• Bilingual in English and Spanish preferred.
• Education or experience in operating a computer required.
• Must be a licensed driver with access to an insured vehicle
