In your Resident Care Coordinator role, you will be responsible for completing weekly scheduling for staff, training new hires, supervising caregivers, supervising the medication delivery system, and servicing needs of existing staff.

Responsibilities :
- Reviews and documents daily in the communication log as appropriate.
- Reviews new orders and MAR daily.
- Supervises and reviews the transcription process, medication and equipment ordering, pharmacy cycle fill.
- Performs resident care coordination and communication with outside agencies and medical providers.
- Reviews alert charting.
- Reviews incident reports as needed.
- Reviews cleanliness and organization of medication room.
- Reviews, supervises, and assists with resident care as needed.
- Performs medication pass as needed.
- Assists residents with meals as needed.
- Reviews and documents on the service plan as needed.
- Attend resident service plans review as needed.
- Reviews and responds to Community R.N. audit to ensure compliance.
- Assist and / or conduct the orientation for new employees.
Requirements :
- QMAP Certification required
- 1yr experience required
- High school diploma or GED preferred
- CPR Certified from American Red Cross or American Heart Association
