Openwork is currently assisting the HAY Center in their search to fill a Housing Readiness Case Manager position. As a Housing Readiness Case Manager you will be responsible for working with youth ages 17.
5 through 24 to complete housing assessments and prepare them for the housing and leasing process. You will also assist them in navigating the voucher application process, complete applications and attend housing briefings.
Responsibilities for the Housing Readiness Case Manager job include :
Job Duties
Completes housing assessments to determine housing readiness and provides case management / supportive services needed to ensure youth are prepared for the housing opportunities available.
Assists youth through the housing process including application completion and voucher process.
- Completes, collects, and submits documentation as needed to the local housing authorities through the initial housing process and recertification.
- Assist youth in the move-in process at the HAY Center Campus
- Provides onsite case management to youth residing on the HAY Center Campus
- Attends and participates in wraparound meetings with youth as needed.
- Completes required data entry and maintains case files for assigned cases.
- Works closely with internal programs with an emphasis on the In-care Program : ensuring youth have information regarding available housing programs and how to prepare for housing requirements.
- Participates in HAY Center / Housing Continuum Events as needed
Qualifications :
- Bachelor’s degree in Social Service, Psychology, Social Work, Criminal Justice or closely related field
- Knowledge of principle, practices and techniques of case management and administration, case management documentation and records management.
- Reliable transportation
- Must pass background check and drug screening
- Experience using Microsoft Office
Preferred Experience
- Some experience working with / knowledge of public housing and housing subsidy.
- Some experience working with / knowledge of transitional needs of youth aging out of foster care.
Perks :
- Typical workday : Monday Friday; Weekends (To be discussed in interview)
- Pay Rate : $22.46 hr.

- Position through May 2025
