Job Description : *
We are seeking experienced and caring Physical Therapists to join our dynamic team of healthcare professionals servicing Hyde Park and the surrounding South side of Chicago.
As a Home Health Physical Therapist, you will play a crucial role in the well-being of our patients, providing one-on-one, in-home therapy services to help them achieve their rehabilitation goals.
Our clinicians are 1099 independent contractors and enjoy the flexibility with caseload and service territory.
- Key Responsibilities : *
- Conduct thorough evaluations to assess patients' physical condition and needs.
- Develop personalized treatment plans to address patients' unique goals and challenges.
- Implement evidence-based therapeutic techniques and exercises to improve mobility, strength, and function.
- Monitor and document patients' progress and adjust treatment plans as needed.
- Educate patients and their families on proper home exercise programs and injury prevention strategies.
- Collaborate with other healthcare professionals, such as nurses, physicians, and occupational therapists, and Physical Therapy Assistants to provide holistic care.
- Qualifications : *
- Bachelor's, Master's, or Doctoral degree in Physical Therapy.
- Current state licensure as a Physical Therapist.
- Home Health experience is a plus but not required.
- Strong interpersonal and communication skills.
- Compassionate, patient-focused, and dedicated to improving patients' quality of life.
