Position Overview
The CHW- Driver is a critical member of the Medically Vulnerable Population (MVP) clinical team. The MVP is an innovative, multi-partner, respite, and recuperative care facility for individuals experiencing homelessness.
It is designed to promote wellness, deliver equitable care, and address social factors impacting patient health. The CHW - Driver will collaborate with Fourth Street Clinic staff, The Road Home, clients, and other partners to provide coordinated transportation and care coordination for the individuals temporarily housed at the MVP.
Core Responsibilities
- Must be able to safely drive the Fourth Street Clinic ADA-accessible van to various appointment sites.
- Coordinate with clients and staff to schedule transportation needs for clients at the MVP.
- Inspect the van and equipment daily for damage, equipment failures, and safety items to ensure the safe use of the equipment and van.
- Completing, in writing, the Daily Driver Vehicle Inspection Report.
- Ability to follow operating procedures-including but not limited to start-up and shut-down procedures for operation of the ADA van.
- Must be safe, courteous, and professional.
- Assist with other client-related care coordination, including client registration, medication delivery, and other tasks.
- Coordinate with site staff on the operation of the outreach transportation.
- Communicate schedule and program changes or adjustments to all internal and external staff.
- Implement, support, and monitor all HIPPA and OSHA guidelines.
- Attend regular scheduled staff meetings. Implement and conduct MVP-related meetings with necessary staff.
- Essential physical requirements : ability to lift 50 lbs. Other physical movements include pushing, balancing, stooping, flexing / twisting, and extended time on your feet.

Qualifications
- High School diploma or equivalent
- Proof of insurability
- Cultural humanity and sensitivity
- Leadership, negotiating, and problem-solving skills
- Valid Driver's license in the state of Utah
- Pass the necessary program background checks
- Good communication skills such as listening, speaking, and writing
- Experience in Excel, spreadsheets, and data
- Ability to work independently and as part of a group
- A safe driver with no more than 2 moving violations in the past 3 years
- No convictions for driving under the influence of alcohol or any drugs in the past 10 years
Preferred Qualifications
- A valid Utah Commercial Driver's License (CDL) class A or B
- Six months of experience driving a commercial vehicle (RV, bus, ambulance, fire truck, or other large vehicle)
- Department of Transportation (DOT) medical card
- A safe driver with no more than 2 moving violations in the past 3 years
- No convictions for driving under the influence of alcohol or any drugs in the past 10 years
