Summary of Role :
The Community Living Coordinator is one of the key support positions for MTCs clients and is responsible for the coordination of staff and clients between the multiple living facilities.
They are to support the staff and ensure clients are cared for in the best way possible to empower them to be successful.
The Community Living Coordinator will also ensure that all regulatory and internal requirements are being fulfilled while helping to elevate and empower our community members.
Specific Responsibilities :
Serves as the primary liaison between consumers, staff members, and other stakeholders (i.e. support coordinators, service providers, and family members) and collaborates with clients, families, and circle of support for best care opportunities.
Ensures compliance with all MTC policies and procedures, requirements of the Agency for Persons with Disabilities (APD) and other regulatory agencies.
- Responsible for managing Personal Support staff, Supported Living Coaches, including interviewing, hiring, motivating, conducting 90-day and annual performance reviews on staff, monitoring schedules, approving timesheets, and any disciplinary actions as needed.
- Ensures a healthy, safe, and comfortable living environment is being promoted by staff for the consumers.
- Ensures collection of consumer data to complete program outcome reports quarterly.
- Reviews & maintains consumer files to ensure required documentation is maintained in compliance with regulatory and auditing agencies.
- Monitors and supports staff to ensure that all service delivery entries in iConnect systems are complete, accurate, and submitted in accordance to established deadlines.
- Performs editing and quality control of iConnect entries in preparation for billing.
- Accurately completes all required Florida Medicaid and APD / iBudget paperwork for each consumer.
- Develops implementation plans with consumer's input and provides needed support to achieve stated goals.
- Ensures community inclusion, medical appointments, prescriptions, transportation, third-party benefits, and other needs are being addressed for each consumer.
- Occasional Night / Evening work may be required.
- All other duties as assigned.
- 2+ of professional experience in working with persons with developmental disabilities.
- 2+ years experience in customer or client service.
- Experience managing employees.
- Excellent communication and writing skills.
Experience Preferred :
- Problem-solving skills and critical thinking skills.
- Time management, highly organized and detail oriented is required.
- Be a self-starter.
Educational / Certification Requirements :
Bachelor's Degree or equivalent years of experience. (Nursing, education, or social, behavioral or rehabilitative science)

iConnect (Billing Software).
Physical / Driving Requirements :
- Must have valid driver's license and insurance coverage.
- Must be able to lift up-to 25lbs.
Travel Requirements :
Must be willing to travel between both locations and drive clients if needed.
Language Requirements :
English proficiency (both written and spoken).
