Job Overview
The Retention in Care Specialist will conduct outreach to patients who have not attended an HIV medical appointment and engage them in HIV care by assisting patients in removing barriers that keep them out of care through the provision of tailored interventions.
The Retention in Care Specialist will provide quality and comprehensive HIV Health Education, HIV testing, and HIV prevention education services to clients at high risk for infection or transmission of HIV.
Core responsibilities include linking clients that have not attended an HIV medical appointment, implementing ARTAS, and maintaining clients engaged into care.
Minimum Requirements
1. High School Diploma required. Bachelor Degree in Science in Health Science with an emphasis in Community Health Education or related field or equivalent work experience preferred.
2. A minimum of 5 years of experience in a community HIV Prevention program or human service-related field, such as social work, psychology, nursing,
3. counseling or health education
4. Trained in Motivational Interviewing and demonstrates a capacity to deliver client-centered counseling.
5. Demonstrated skills in Presentation and Group Facilitation.
6. Applicant must demonstrate skill (through education or experience) in counseling, crisis intervention, critical reading and writing as it relates to implementation of program goals and objectives.
7. Bilingual : English / Spanish required.
