TITLE : Pharmaceutical Care Specialist

What we're looking for :
- Pharmacy doctorate (PharmD) required
- Minimum of two years experience in a hospital or similar health system
Hours and compensation potential :
- This is a full-time position
- Salary range between $K to $K, depending on experience
- Full benefits package included. Relocation assistance may be available.
Responsibilities :
- Provide general clinical pharmacy services, including D.U.E. development and reporting, teaching opportunities, and administration of research protocols
- Compound and dispense medications to patients
- Provide drug information and education to optimize quality of care
- Orient new personnel or students to department services and programs
- Provide educational in-service programs to share latest treatments and therapies with care providers
- Contact prescribers to clarify medication orders or notify of unobtainable medications to reduce errors and therapy delays
- Assist in inventory management and ordering to minimize stock outages
- Support formulary practices and procure quality generic products
- Maintain medication on assigned patient care areas
- Prepare emergency carts and boxes for adequate availability of in-date medication
- Calculate caloric and electrolyte ingredients for TPN administration
- Provide timely and accurate drug and toxicological information
- Accurately perform medication order entry
- Monitor drug therapy and screen for potential medication problems
- Apply pharmacokinetic / pharmacodynamic principles to individualize drug dosing
- Respond to and support Code Blue and Code Trauma situations
- Adjust doses for patients with diminished renal or hepatic function
- Individualize medication doses for neonatal, pediatric, and geriatric patients
- Calculate and compound chemotherapeutic agents for cancer patients
- Recommend formulary positions / approaches to reduce costs and improve care
- Utilize spreadsheet and clinical based computer programs
- Assist in administering approved investigational drug research protocols
