Adult & Child Health is an integrated healthcare provider in Central Indiana. We are a Certified Community Behavioral Health Center and a Federally Qualified Health Center look alike;

we strive to provide the best in healthcare services. Providing high-quality, affordable care including primary care, behavioral health services, substance abuse treatment, mobile crisis response, community-based services, homeless outreach, school-based treatment, and more.

About this role : As an SSVF Case manager you will provide assessment, engagement, and referrals for veterans experiencing or at risk of homelessness.

In this position you will assist veterans in eliminating barriers that may lead to or keep persons in homelessness.

The Location and hours : A&C is based in Central Indiana. For this position, we service the following county : Marion

Hours are M-F 9-5pm with flexible scheduling available.

Responsibilities include but not limited to :

  • Providing collaborative planning, referral and monitoring of treatment and support services.
  • Connecting persons with resources to address immediate needs including but not limited to : primary care, home healthcare, mental / behavioral healthcare treatment, entitlements, and supportive employment resources.
  • Engaging persons in transition to housing.
  • Collaborating and coordinating with community partners, referrals sources, providers and advocating on behalf of persons being served in the project.
  • Completing required documentation and data entry in a clear, accurate, and timely fashion.

Requirements :

  • Strong working knowledge of community resources
  • Bachelor’s degree in Social Work, Psychology or related field, required
  • Experience working with individuals experiencing homelessness
  • Available to work weekdays and some evenings
  • Strong organizational, communication, and writing skills
  • Must hold a valid driver’s license with acceptable driving record and provide own, insured, transportation
