POSITION OVERVIEW :
The Case manager, under the supervision of the Supportive Housing Manager, is responsible for providing case management, housing support to participants and monthly reporting requirements that will ensure compliance with the parameters of the program.
Pays : $50,000 per year
MINIMUM QUALIFICATIONS
Education : Associates degrees, and
Experience :
- Two years of experience working in a scattered site or congregate care level setting of supportive housing program, the capacity of or case management or service coordinator.
- Experience working with Substance Use, Mental Health and homeless population
Education : High school diploma, and
Experience :
- Five years minimum Ten years ’ experience of experience working in a scattered site or congregate care level setting of supportive housing program, the capacity of or case management or service coordinator.
- Experience working with Substance Use, Mental Health and homeless population
KEY ESSENTIAL FUNCTIONS :
- Case Management and Record Keeping
- Provides case management and on-site crisis intervention
- Completes required service plans.
- Facilitates monthly CAB tenant meetings.
- Educates participants with respect to good housekeeping and home-safety practices, budgeting and landlord / tenant relations, as necessary
- Maintains appropriate documentation of all tenant contacts.
- Documents monthly progress notes in the participants’ chart
- Conducts home visits to help evaluate participants’ compliance with the rules and regulations of the program and to assess the need for referral to supportive services
- Takes random urine samples for drug screening toxicology
- May escort participants to social services agencies, as requested
- Reports any alleged child neglect or abuse to the Supportive Housing Manager.
- Housing Reporting Requirements
- Responsible for completing annual tenant rent calculation forms.
- Responsible for completing annual apartment inspection forms.
- Updates monthly program database.
- Ensures annual leases are updated and filed.
- Housing Liaison
- Acts as liaison between participants, superintendents and building managers to ensure all repairs are completed to either the participant’s apartment unit or the building.
- Maintains a Building and Apartment Log , which includes date, and condition of the apartments and buildings.
- Reports all non-compliance issues with respect to the apartment and building to the Supportive Housing Manager, including incidents requiring immediate attention.

Job Requirements
- Knowledgeable of the treatment philosophy, practices, policies and procedures of Acacia Network, particularly with respect to SPC services and operations, including HUD guidelines
- Knowledgeable of the concepts of relapse prevention, including case management techniques, group dynamics and terminology
- Knowledgeable of chemical dependency, including types of abused substances and the pharmacology of abused drugs
- Knowledgeable of resources available, either within the agency or by referral, to meet participant needs
- Thoroughly knowledgeable of client confidentiality rules and regulations and participant rights
- Able to facilitate groups and provide leadership skills to participants
- Able to establish rapport and productive relationships with participants
- Able to inspire confidence, determination and trust in participants and motivate participants to adopt positive values and behavior
- Able to work constructively with a wide range of staff
- Able to meet the agency’s standard of behavior.
