Community Engagement Specialist Full Time - Exempt Columbus, OH AndHealth is a healthcare technology company c reated to help people reverse chronic disease.

We are seeking a Community Engagement Specialist who will engage in testing activities, community outreach, and high-touch individualized support.

The Community Engagement Specialist will collaborate with community partners, Community Health Center (CHC) partners, at-risk individuals, and clinical and medical staff to identify and appropriately link patients to appropriate care and resources.

The Community Engagement Specialist will also provide culturally sensitive one-on-one support and guidance to patients with social determinants of health (SDH) needs.

What you’ll do in the role : Conduct HIV, Hepatitis B, Hepatitis C, and other STI screening / testing. Collect blood, urine, and other appropriate samples to complete the testing.

Identify, engage, and counsel high-risk patient populations. Develop community relationships and partnerships to assist in identifying at-risk populations.

Educating on risk reduction strategies to reduce transmission of infectious diseases. Develop and provide educational presentations regarding disease prevention, intervention, testing, and treatment.

Provide basic education on HIV, Hepatitis B, Hepatitis C and other STIs. Provide test results to patients, and providing appropriate advice and support to individuals identified to have an infectious disease.

Document all actively and submit required reports as required. When necessary, link patients to appropriate health care provider or supportive service.

Provide support to Nurse Practitioners during onsite clinic days. Addresses identified patient SDOH needs and improves patient access to care and community resources.

In collaboration with clinical staff, links patients to essential support services including housing, nutrition assistance, utilities, transportation, and other needs as identified by the patient or healthcare team.

Education & Licensure Requirements : Phlebotomy certification preferred. Other Skills or Qualifications : Past experience with intervention or prevention services preferred.

Knowledge of community resources. Knowledge of HIV / AIDS, Hepatitis B, Hepatitis C, and / or STIs. Effective customer service, communication, and interpersonal skills required for interaction with patients, families, and community providers.

Ability to work as a part of a team, with diverse populations, (i.e. racial / ethnic, socioeconomic and educational backgrounds), and with healthcare providers in clinical settings.

Ability to work in stressful environments, with high-risk populations and people experiencing crisis. Strong attention to detail and organizational abilities required.

Sound decision making skills, with the ability to maintain highly confidential information. Moderate computer skills required (familiarity with Microsoft Word and Excel, required).

Ability to prioritize tasks, problem solve, and work autonomously. Valid Driver’s license. Ability to travel and work flexible hours, which may include some evenings and weekends.

Health Center Requirements : (may be required) Remove if not applicable. (PPD) TB Skin Test - Proof of negative TB skin test within the last 12 months.

MMR) Measles, Mumps and Rubella or A Blood Titer proving immunity. Varicella - (2) documented doses or A Blood Titer proving.
