Summary :
The Adult Outreach Manager is responsible for providing clinical and administrative support to the Integrated Health Home (IHH) Teams and Assertive Community Treatment Teams that provide community-based services and supports to adults residing in the community who are living with a severe and persistent mental illness.
In addition the manager will work with both teams to monitor and provide medically necessary interventions to assist in the management of symptoms of illness as well as overall life situations including accessing needed medical social educational and other services necessary to meeting basic human needs.
LAdult Outreach Manager will promote a collaborative team approach to meet the overall needs of the clients and support the team managers.
Responsibilities :
Ensures that required screenings and assessments of clients are completed through the person-centered treatment planning process assisting and training as needed;
uses information to assist staff in determining client goals and needed services.
Assists Director in supporting the implementation of identified evidence-based practices or treatment options associated with behavioral health disorders.
Supports managers and staff in identifying resources natural supports and developing goals and objectives to use them effectively.
Guides the development of and training in strategies for engaging families in treatment and involvement in supporting the client.
Assists team managers in developing relationships with other providers for care coordination particularly health care providers.
Provides oversight of the processes of admissions and discharges of clients including program eligibility engagement in treatment appropriate level of care coordination of care and discharge planning.
Provides supervision to team managers that includes coordinating team conferencing department and agency meetings.
Assists team managers in monitoring the detailed accurate and timely documentation in health records to meet licensing standards strategizing with the managers on ways to support staff in meeting expectations.
Enhances professional expertise through educational activities and trainings.
Assists Director in monitoring financial targets and making adjustments to programming and expenses as needed to minimize variance to budget.
Works with the finance / billing department to resolve authorization and billing issues.
Assists Director and team managers in guiding staff to meet their requirements for data collection client contact time according to treatment plan and for agency productivity targets.
Assists Director in completing administrative tasks of approving payroll mileage benefit time etc.
Follows agency policies and standards for client rights and confidentiality.
Hires orients trains and evaluates Team Managers / staff according to Gateway Healthcare policy.

QUALIFICATION REQUIREMENTS :
Masters degree in a clinical field (mental health social work marriage / family therapy)
LMHC LICSW LMFT or masters level Licensed Chemical Dependency Professional (LCDP)
At least 3 years experience working with in a behavioral health or human service setting.
Experience in developing and / or coordinating programming with the targeted population.
NECESSARY SPECIAL REQUIREMENTS :
Knowledge of human development and family systems issues.
Understanding of mental health and substance use diagnoses
Knowledge of community resources for clients and families with behavioral health disorders.
Advocacy and coaching skills.
Valid driver's license and use of in order to perform the major functions of the job.
Automobile liability insurance coverage for vehicle being used in accordance with Center requirements.
Ability to work collaboratively as a team to coordinate care for clients with intensive behavioral health needs.
