The Clinical Pharmacist works to support the mission and vision of Neighborhood Healthcare by providing excellent patient-centered care and services within a dynamic refill team environment.
This role will utilize pharmacist skills such as assessment, critical thinking, judgement, decision making in reviewing and filling patient prescriptions.
Additionally, this role will provide cross-cover support in the retail pharmacy, including site and personnel oversight.
Essential duties and responsibilities
- Examines problem lists and medication lists to determine if there are contraindications or significant interactions, and make appropriate recommendations for change
- Substitutes therapeutically equivalent medications in accordance with health plan formularies independently
- Clarifies drug interactions, directions for use, and substitutions in concert with external pharmacists, and notifies provider when appropriate
- Answers clinical questions from providers accurately, promptly, and thoroughly using the most up to date clinical guidelines and evidence
- Provides retail pharmacy support and oversight, as needed
- Provides clinical oversight and direction of non-pharmacist staff, as needed
- Attends and participates required meetings and trainings
- Performs all job functions with Neighborhood’s mission, vision, values, and strategic plan in mind
- Learns and navigates Neighborhoods electronic medical record (EMR) to investigate refill requests initiated by patients and pharmacies
- Communicates via the EMR with other Neighborhood staff as it relates to patient care in accordance with the Neighborhood Pharmacist-Nurse Refill Protocol
- Refills medications on behalf of providers according to Neighborhood’s Pharmacist-Nurse Refill Protocol
- Orders labs needed to monitor medications with the Neighborhood Pharmacist-Nurse Refill Protocol
- Orders outstanding routine labs required for health maintenance
- Reviews telephone encounters to identify duplicate or missing medications on patient medication lists
- Contacts patients and pharmacies to clarify medication discrepancies prior to refilling a medication
- Maintains accurate and complete records in the electronic health records of all patient encounters

- Contributes to the success of the organization by participating in quality improvement activities
- Cooperates in accomplishing department goals and objectives
Customer Relations :
- Responds promptly and with caring actions to patients and employees
- Maintains professional working relationship with all levels of staff, patients, and the public
EDUCATION / EXPERIENCE
- Pharmacy doctorate degree from an ACPE accredited pharmacy school required
- Valid CA pharmacist license required
- Board Certification Pharmacotherapy Specialist or other specialty preferred
- Current BLS certification required
- Pharmacy practice residency preferred
- Bilingual (English / Spanish) or (English / Arabic) preferred
ADDITIONAL QUALIFICATIONS (Knowledge, Skills, and Abilities)
- Excellent verbal and written communication skills, including superior composition, typing, and proofreading skills
- Ability to interpret a variety of instructions in written, oral, diagram, or schedule form
- Knowledgeable about and experience with pharmacy practices and protocols
- Ability to successfully manage multiple tasks simultaneously
- Excellent planning and organizational ability
- Ability to work as part of a team as well as independently
- Ability to work with highly confidential information in a professional and ethical manner
Physical Requirements
- Ability to lift / carry 20 lbs / weight
- Ability to stand for long periods of time
COMPLIANCE (Safety and HIPPA)
- Maintain current knowledge of policies and procedures as they relate to safe work practices
- Follow the NHC Pharmacist-Nurse Refill Protocol for refill authorizations
- Follows all safety procedures as outlined in Neighborhood Healthcare’s Illness and Injury Prevention Plan (IIPP) and report any injuries and / or unsafe conditions immediately
- Maintains current knowledge of policies and procedures as they relate to safe work practices
- Follows all safety procedures and report unsafe conditions
- Uses appropriate body mechanics to ensure an injury free environment
- Familiarity with location of nearest fire extinguisher and emergency exits
- Follows all infection control procedures including blood-borne pathogen protocols
- Maintains privacy of all patients, employee and volunteer information and access such information only on a need-to-know basis for business purposes
- Complies with all regulations regarding corporate integrity and security obligations
- Reports all behavior and / or activity that are unethical, fraudulent, or unlawful
Pay Range : $140,000 to $160,000 per year, Depends on Experience (DOE)
