American River Home Care is looking for a motivated and experienced Physical Therapist Assistant to join our thriving team as a Rehab Care Coordinator.
JOB DESCRIPTION SUMMARY
The Rehab Care Coordinators primary role involves supporting the daily operations of therapy services, including coordinating home health care visits and managing administrative tasks to ensure seamless delivery of care.
At times you will be responsible for providing direct patient care when needed.
CLINICAL / OPERATIONAL RESPONSIBILITIES :
1. Assists the Clinical Therapy Manager with daily office tasks such reviewing referrals, patient care audits, scheduling appointments, managing patient records, and coordinating communication with healthcare providers.
2. Coordinates and schedules home health care visits, ensuring timely and efficient delivery of therapy services to patients.
3. Serves as a point of contact for patients, caregivers, and healthcare professionals, addressing inquiries and coordinating care plans.
4. Supports therapy staff by preparing necessary documentation, tracking visit schedules, and ensuring compliance with regulatory requirements.
5. Collaborates with the therapy team to optimize patient outcomes, streamline processes, and enhance the quality of care.
6. Participates in quality improvement initiatives and contributes to the continuous improvement of therapy operations.
7. Maintains confidentiality of patient information and adheres to ethical and professional standards in all interactions.
8. Provides direct physical therapy according to the directions of the physical therapist and the physician’s plan of care.
9. Participates in public relations and community activities that promote the organization’s role in the healthcare delivery system.
10. Perform additional duties and responsibilities as assigned, adapting to the needs of the department to support overall operations and ensure the highest quality of service.
POSITION QUALIFICATIONS
1. Graduate of an approved Physical Therapy Assistant program.
2. Current certification or licensure as required by state law.
3. Maintains current CPR certification.
4. Strong communication, organizational, and problem-solving skills.
5. Licensed driver with an automobile that is insured per state / organization requirements and is in good working order.
6. Compassionate attitude, empathy for patients, and a commitment to delivering high-quality care.
