overview
As a Speech Therapist youwill be responsible for providing speech therapy services to Agencypatients as prescribed by the attending physician and for providingAgency staff and the patients physician with speech therapyconsultation as needed.
You will assess plan and implementrehabilitation services for speech language and / or swallowingdisorders and maintain adequate clinical and progress notes.
Additionally you will be required to drive daily for meetingsand / or visits to residential homes and work in varied residentialenvironments based on assigned caseload.
Key ResponsibilitiesAssess plan and implement rehabilitation services for speechlanguage and / or swallowing disorders. Provide evaluation of speechand language abilities through diagnostic testing.
Establish goalsand estimate prognosis for recovering. Record and reports theclients progress in treatment and any change in patients conditionto the physician.
Instruct family member(s) and members of thehealth care team in methods of assisting the patient in improvingcommunication skills.
Reevaluate patients progress and theestablished rehabilitation program as often as necessary. Prepareperiodic reports as required by the Agency which are sent to thephysician and third party payers regarding care being provided.
Assist in making appropriate referrals to outside audiology speechclinics or other community services as indicated. Maintain adequateclinical and progress notes and submits all required paperwork in atimely manner according to Agency policy.
Provide liaison contactand followup with other community agencies. Participate ininservice case conferences and staff meetings as required.
Maintainand updates professional competency through inservice attendanceand similar professional resources. All other duties as assigned bysupervisor.
Schedule and Shift Details The work schedule and shiftexpectations are based on assigned caseload. Employee will berequired to drive daily for meetings and / or visits to residentialhomes.
Employee will work in varied residential environments.Travel Employee will be required to drive daily for meetings and / orvisits to residential homes.
