MDS Coordinator Job Summary :
As the MDS Coordinator, you'll serve as the Resident Assessment Instrument (RAI) / Minimum Data Set (MDS) Coordinator for our center.
Your responsibilities will include reviewing and processing MDS assessments in accordance with CMS and LTCI regulations and guidelines.
MDS Coordinator Responsibilities :
- Coordinating the development and revision of resident Interdisciplinary Care Plans based on RAI / MDS information
- Collaborating with nursing caregivers and other disciplines to ensure accurate care plan goals
- Effective communication with various disciplines to facilitate timely completion and transmission of RAI / MDS
- Preparing accurate reports on Quality Measure / Quality Indicators (QM / QI) from RAI / MDS data
- Active involvement in Quality Improvement initiatives
- Implementing educational plans to address changing program or service needs
