JOB SUMMARY AND QUALIFICATIONS :
The Targeted Case Manager’s primary duty is to provide case management services to families involved in the Family Recovery Program in Daviess County, KY.
The Case Manager will be responsible to assist the individual / family with obtaining needed access to housing, medical, social, educational and other identified services.
WHAT YOU SHOULD HAVE FOR THIS ROLE :
Bachelor’s degree in a behavioral science (including psychology, sociology, social work, family studies, human services, counseling, or another approved human service degree).
- A minimum of one (1) year of full-time employment working directly with adolescents or adults in a human service setting;
- preferably in the field of Substance Use; (2) successful completion of case management training approved by the Kentucky Department for Behavioral Health, Developmental and Intellectual Disabilities (KBHDID) and completion of recertification requirements approved by KBHDID every three year (TCM CORE and TCM SUD training) (3) knowledge and skills in the areas of case management, crisis counseling, community resources, advocacy, problem solving, critical thinking, communication, mental health, substance use, and documentation;
Master’s degree in a behavioral science preferred.
RESPONSIBILITIES :
1 Provide case management services for families that targeted on the identified needs of the individual and / or family.
2 Conduct comprehensive assessments, and reassessments, to determine the need for housing, medical, social, educational, and other identified services.
3 Develop an individualized case plan for each individual and / or family that is based on the assessment. The plan will specify the goals and activities that address housing, medical, social, educational, and other services identified.
4 Refers the individual and / or family to services that address the areas identified on the assessment. This may include scheduling appointments for the individual / family and ensuring transportation is available for appointments.
5 Monitor and follow-up on goals and activities that are established on the case plan. This may include follow-up with the individual, family members and other service providers.
Update case plan as needed.
6 Identify community resources and develop cooperative relationships with other social service providers.
7 Represent agency programs, policies and procedures accurately in the community and with other social service agencies.
8 Maintain accurate case records, which includes proper documentation of all meetings with clients.
9 Ensure that all documentation complies with KY Medicaid and accreditation standards.
10 Participate in weekly case conferencing and provide updates on all assigned individuals and / or families.
11 Participate in bi-monthly supervision with TCM billing Supervisor.
12 Participate in monthly staff meetings and lead in trainings / discussions as assigned.
13 Attend annually required 10 hour SUD training to maintain specialty certifications.
14 Maintain certification requirements for TCM pursuant Kentucky Board regulations.
15 Assist with special projects as assigned.
16 Assist with reporting requirements as assigned.
