Position Summary : We are currently looking for a Home Care Registered Nurse to join our team of healthcare professionals in Schoharie County, New York.

The ideal candidate will have a passion for delivering patient-centered care and be committed to improving the well-being of individuals in the community.

Responsibilities :

• Conduct comprehensive assessments of patients' health status and develop personalized care plans.
• Administer medications, perform wound care, and provide other skilled nursing services in accordance with physician orders.
• Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure coordinated and effective care.
• Educate patients and their families on managing health conditions and promoting a healthy lifestyle.
• Monitor and evaluate patients' progress, adjusting care plans as necessary.
• Maintain accurate and up-to-date patient records in accordance with regulatory standards.
• Provide emotional support and compassionate care to patients and their families.

Qualifications :

• Current and valid RN license in New York State.
• Minimum of 1 year of nursing experience, with a preference for home care or community health experience.
• Strong clinical and assessment skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a collaborative team.
• CPR certification.
• Reliable transportation and a valid driver's license.
