Over 10,000 homeless individuals are cared for by Boston Health Care for the Homeless Program each year. We are committed to ensuring that every one of these individuals has access to comprehensive health care, from preventative dental care to cancer treatment.

Job Summary :

The Case Manager coordinates effective discharge planning and addresses patients' social service needs. This includes conducting a biopsychosocial assessment within 24 hours of admission to evaluate each patient's needs.

The role also involves assisting with obtaining identification, arranging transportation, and connecting patients to community resources.

The Case Manager works closely with the clinical team to ensure smooth discharge transitions and builds strong relationships with external agencies for ongoing patient support.

This position requires experience working with clients facing mental health challenges, PTSD, and substance use, utilizing a trauma-informed approach.

Knowledge of local shelter systems and financial benefits is essential for timely assistance, particularly when helping patients transition from acute care to long-term solutions like rest or nursing homes.

Strong team communication and collaboration are critical to comprehensive care and support throughout the patient's recovery.

Hours : Full-Time; 40 hours per week, Monday-Friday, 8 : 00am - 4 : 30pm

Responsibilities :

  • Discharge Planning : work as part of a multidisciplinary team responsible for coordinating effective discharge planning.
  • Communicate daily with the team to ensure smooth patient transitions.
  • Documentation : Maintains clear and accessible documentation in the medical record, ensuring the case management plan, including discharge placement, is readily available to the team.
  • Patient Resources : Provides patients with resources to help break cycles of homelessness, addiction, and mental illness, supporting their journey to stability and recovery.
  • Discharge Placement : Secures appropriate discharge placements and, when possible, explores alternatives to shelter living, prioritizing long-term solutions like rest or nursing homes.
  • Problem-solving : Collaborates with team members to develop effective plans for patients exhibiting challenging behaviors, ensuring that these are addressed with empathy and professionalism.
  • Community Connection : Establish strong relationships with community agencies to secure vocational rehabilitation, mental health support, and substance use treatment.
  • Communication & Reporting : Reports all discharge planning progress to the clinical team and Supervisor of Case Management.

Ensure patient discharge dates are communicated to the appropriate team members.

  • Team Contributions : Actively participates in team rounds, providing updates on discharge plans and contributing to patient care discussions.
  • Discharge Plan Development : Continuously builds on the discharge plan week-to-week, demonstrating progress in team discussions and notes.
  • Supervision & Team Meetings : Meets regularly with the supervisor as directed and participates in the weekly case management team meeting to ensure alignment and support for ongoing care initiatives.
  • Collaboration : works closely with the unit secretaries around transportation to medical appointments.
  • Connection to transportation resources, including PT1, the Ride, TAP Pass / Charlie card, or any other necessary medical transportation support;

accompany to medical appointments when it is necessary.

Qualifications :

  • Previous experience in case management or human services is required.
  • Communication Skills : Strong communication, documentation, and organizational skills are essential for adequate patient support and teamwork.
  • Education : A bachelor’s degree, BSW (Bachelor of Social Work), or a related social services degree is preferred.
  • Language Skills : Fluency in Spanish, Haitian Creole, or Portuguese would be helpful to support the diverse clientele at BHCHP.
  • Flexibility and Multitasking : Ability to adapt, multitask, and prioritize responsibilities in a dynamic environment.
  • Stress Management : Capable of working in a fast-paced setting and managing stress positively and proactively.
  • Workflow Management : Ability to create and implement a case management workflow that addresses client needs promptly.
  • Commitment to Underserved Populations : Demonstrated passion for and commitment to supporting underserved populations.
  • Cultural Competency : Understanding diverse cultural backgrounds and experience in delivering trauma-informed care.
  • Substance Use Disorder Knowledge : Experience working with clients struggling with substance use disorders is essential.
  • Internal support of team members : able to assist in training and onboarding new staff as needed.

Compensation and Benefits :

  • The compensation starts at $22.07 per hour and increases based on years of experience.
  • BHCHP full time employees are eligible for our competitive time off policy of 4 weeks’ vacation , health, dental and vision insurance, 403B retirement savings plan and employer retirement contribution, and pre-tax MBTA pass program with 40% discount.

In addition, eligible employees will receive yearly increases, additional compensation of seven thousand five hundred added to your base hiring rate for demonstrated bilingual proficiency and the opportunity to work with local hospitals and community health centers.
