Job Responsibilities

This position is responsible for providing assessment and creating unique interventions to support young people who deserve an opportunity to experience a sense of normalcy and community life.

The applicant must demonstrate an ability to relate to and build positive connections with children and adults, be comfortable working in a strength-based and family-centered way and be able to work effectively within a team environment.

Coordinators must demonstrate an ability to effectively communicate with families, Child and Family Teams members (stakeholders, therapists, case managers, etc.

and agency management.

Familiarity with Child and Family Teams and ability to provide clinical supervision is also required. In addition candidates need experience working with children and families as a Care Coordinator, Intensive Case Manager, Clinical Liaison, Supervisor, or Team Lead with experience in home and community-based support services.

Other key functions of this position include : facilitating team meetings, performing family intakes and writing assessments / support plans, measuring progress and outcomes of services provided, promoting employee development and evaluating performance.

Flexibility in scheduling, willingness to drive to different cities, and work within the homes / communities of the families we work with is required.

Requires afternoon, evening, and weekend work.

This position will also participate in an on-call rotation.

Education / Experience Needed :

High school diploma or GED

Minimum of five (5) years related work experience or Related Bachelor's degree

Supervisory experience preferred.

Bilingual (Spanish) a plus!

Requirements :

Must be 21 years or older

Valid Arizona Driver's Licence with an acceptable 39 month driving record

Reliable transportation, including proof of Insurance

Candidates are required to submit a negative drug test

Obtain an Arizona DPS Level One Fingerprint Clearance Card

Complete Adult, Child, and Infant CPR and First Aid training
