SUMMARY :
The purpose of the care coordinator is to develop a plan of care for each family and their stakeholders. Care Coordinators should meet with their individuals, conduct a strengths-based inventory, convene the individual's team, and develop the care or treatment plan based on the individual's needs, goals (formal and informal), and resources available or needed by the individual.
DUTIES AND RESPONSIBILITIES :
The Care Coordinator shares a caseload of no more than 10 individuals : Develops an individualized Plan of Care (POC) based on the identified strengths and needs, including a comprehensive 24-hours Crisis Plan :
- Monitors the provision and quality of services provided to the individual through the Team and is the liaison when new services / resources need to be sought or developed.
- Provides or secures support and crisis / emergency services to the individual through face-to-face contact, phone contact and availability by on-call system.
- Completes all the necessary paperwork in a strength-based manner per South Carolina and BBH requirements.
- Collaborates with other necessary individuals the individual and family may have contact with, such as Case Managers, Probation Officers, Judges,
- Provides written and verbal information related to the individual's behavioral and mental health needs and progress.
- Other duties as assigned
QUALIFICATIONS :
- Care Coordinators must possess a bachelor's degree (master's preferred) in a relevant area of education or human services and a minimum of 3 years of continued experience providing mental health services.
- Meet all licensing requirements set forth by the respective state of operation;
- Meet the background check requirements for each state's regulations;
- Successfully pass an initial medical examination and pass a drug screening, which provides evidence that they are free from communicable diseases, including active tuberculosis or conditions which prohibit them from reasonably performing the assigned duties, except as provided for by the ADA.
Staff will be reexamined on a regular basis.
