The Program Enrollment & Outreach Manager is responsible for driving program enrollment for PACE. This position develops and executes an enrollment growth plan, participates in community awareness and outreach opportunities, and ensures the enrollment process is efficient and easily accessible for potential enrollees.
RN Position Duties & Responsibilities :
- Conducts home-based assessments of potential enrollees. Evaluates potential new enrollees, determines levels of care, and communicates with the appropriate team members and / or regulatory services.
- Serves as primary liaison for the intake process for potential participants. Collaborates with inpatient Case Management staff, outpatient departments, physicians, physician liaisons, community referral sources and PACE staff in assessing, identifying and navigating intake process.
- Develops and executes a strategic plan to increase program enrollment for PACE.
- In close partnership with Marketing and Outreach Teams, identifies target audiences and relays key information about the PACE program.
Coordinates educational events for community groups.
Builds awareness among referral sources and healthcare community partners. Identifies outreach opportunities to the healthcare community;
maintains strong relationships and referral patterns among key medical professionals.
- Identifies and tracks issues, trends and opportunities. Explores efficiencies in PACE enrollment process and solutions to barriers for enrollment.
- Serves as a subject matter expert for Medicare / Medicaid; answers questions and explains processes in plain language for potential participants.
Assists potential enrollees with the Medicaid application process and follows the process to completion.
Other projects and duties as assigned.
RN Position Qualifications :
- RN with current RN licensure in the state of Kentucky.
- Minimum two years nursing experience with geriatric population.
- Working knowledge of Medicare and Medicaid processes, payments, and requirements strongly preferred.
- Valid Driver’s license and an acceptable driving record required.
- Excellent communication skills, with an ability to adapt communication style to a wide variety of audiences and strategies.
- Strong attention to detail and accuracy, excellent organizational skills with ability to prioritize, coordinate and multi-task with high level of quality and productivity.
- Strong analytical and problem-solving skills.
- Flexibility to travel throughout the PACE service area.
