Overview

The Passage of Hope (POH) HS / PRS Program provides case management and other supportive services to Unaccompanied Children (UC) who are currently under legal custody of the Federal Government or who have completed the reunification process and are now residing in the community with their sponsors.

Working with the federal Office of Refugee Resettlement, our HS / PRS program provides support and assistance to families in need of Home Study Services and Post Release Services (PRS).

For PRS Services we link UCs and their families with an array of community services that will help them achieve success and permanency in the United States, so they are able to move forward with their lives and thrive.

Our PRS program operates across three distinct levels, each tailored to address the unique needs of released UCs and their sponsor families.

Level One (1) PRS focuses on confirming stability and promptly addressing any emerging issues to ensure a smooth transition post-release.

Level Two (2) PRS offers comprehensive case management services over a six-month period to monitor progress, provide psychoeducation, and connect UCs and sponsors to vital community resources tailored to their specific needs.

At Level Three (3), PRS Community-Based Clinicians provide intensive in-home or community-based services and engagement to released UCs and their sponsor families.

These clinicians utilize a trauma-informed, intensive case management approach to support families facing specific challenges or special circumstances.

The role focuses on stabilizing family units by tailoring clinical interventions to meet unique family needs and connecting them with community-based services for long-term care, including mental health support.

Responsibilities

  • Implement a trauma-informed approach in all interactions and interventions with released UCs and sponsor families Utilize evidence-based trauma interventions, such as Trauma-Focused Cognitive Behavioral Therapy (TF-CBT), to address trauma-related needs effectively.
  • Conduct comprehensive assessments within 7 days of release to ensure the safety, well-being, and specific needs of each UC.

Follow up with routine clinical assessments to update treatment goals as necessary.

  • Develop and implement crisis intervention strategies and safety plans for UCs and their families during emergencies or critical situations to ensure immediate support and stability.
  • Engage with stakeholders involved in the UC’s case, including schools, legal service providers, and extracurricular activities, to ensure individualized services targeting specific needs and to deliver individualized services.
  • Connect sponsors and UCs to community resources and coordinate essential services related to placement, stability, immigration proceedings, guardianship, legal assistance, education, medical care, mental health services, family counseling, substance abuse support, and gang prevention as needed.
  • Maintain thorough and accurate documentation of all engagements, assessments, services provided, and outcomes via PRS report forms, ensuring compliance with ORR guidelines and specified timeframes.
  • Complete Notice of Concern (NOC) forms for any concerns about the child's safety and well-being, make notifications to appropriate investigative agencies, and notify ORR within required timeframes.
  • Ensure compliance with core competencies and training requirements related to child welfare, ORR, and trauma-informed care.
  • Collaborate effectively with other professionals in the field, staying informed about issues affecting UCs and best practices in trauma-informed care.
  • Participate in and attend all required program-related meetings and conferences to contribute to program development, collaboration, and continuous improvement efforts.
  • Provide evening and weekend coverage as needed to ensure continuous support and availability for UCs and their families during critical times, including participating in on-call duties outside regular business hours
  • Perform other duties as assigned to contribute to the overall success and effectiveness of the HS / PRS program in supporting released UCs and their sponsor families.

Qualifications

  • Master’s degree in social work, psychology, sociology, or relevant behavioral science with direct clinical experience as a program requirement or a bachelor's degree with at least five years of clinical experience.
  • Must be licensed or eligible for licensure in the relevant field (., LMHC, LMSW).
  • At least two years' experience working with youth in the social service field.
  • Highly proficient in Spanish or have access to qualified interpreters for effective communication.
  • Demonstrates expertise in trauma-informed care, crisis intervention, and family-centered interventions.
  • Knowledge of child welfare and immigration related legislation
  • Excellent verbal, written communication and organizational skills are essential
  • Skilled in Microsoft Office applications, including Word, Excel, and PowerPoint.
  • Experienced with electronic health records (EHR) systems (v).
  • Bilingual (English / Spanish) a must.
