Community Health Workers (CHWs) serve as liaisons between healthcare providers and the community, promoting access to care and health education.
Key Responsibilities :
- Assist individuals in navigating healthcare services and resources.
- Provide education on health topics and disease prevention.
- Conduct outreach and support community health initiatives.
- Collect data on community health needs and barriers to access.
- Advocate for patients and connect them with necessary services.
Requirements
- High school diploma; relevant certification or degree preferred.
- Experience in community outreach or healthcare settings.
- Strong communication and interpersonal skills.
- Cultural competence and understanding of community dynamics.
