Fairfax has three unique campuses : Kirkland : Is our main -bed, standalone psychiatric hospital, composed of six units providing specialized treatment for mental health and co-occurring disorders, as well as detox services for both adolescents and adults.
Everett : Is our 30-bed adult general psychiatric unit located on the 7th floor of the Providence Medical Center’s Pacific campus.
Monroe : A 34-bed unit on the campus of Evergreen Health Monroe. We are seeking a Full Time (Mon-Fri) Case Manager. Our Case Managers facilitate the development of discharge plans for all patients starting from the moment of admission.
They meet 1 : 1 with patients to collaborate with their treatment and discharge planning; consult with families on the emotional, social, and financial aspects of the patient’s life;
and assist with family meetings with patients (when warranted). They also play a valuable role in multidisciplinary team meetings, patient care conferences, and treatment planning.
Case Managers comply with required standards for medical / legal reporting situations (e.g., child abuse, dependent adult abuse, duty to warn, etc.
