JOB SUMMARY : Responsible to engage relative and non-relative caregivers by delivering family support services. Involves a broad range of training and development activities, assessment services, and consultation and requires quality documentation of services provided.

Participates in and provides leadership in the organization’s continuous quality improvement program.

ESSENTIAL FUNCTIONS :

1. Engages and Introduces Level 1-Licensing for Foster Homes, Guardianship Assistance Program and Kinship services to relative and non-relative caregivers referred to CHN.

2. Makes contact with families by phone or face to face within 48 business hours of referral to CHN.

3. Educates families and community partners on Level 1-licensing, Guardianship Assistance Program and Kinship Services.

4. Responsible for quality documentation and tracking of families served.

5. Maintains timely contact with all families on active caseload by means of home visits, office visits, phone contact or correspondence.

6. Facilitates coordination with assigned Child Placing Agency (CPA) licensing staff for relative caretakers regularly to support completion of licensing requirements within 60 days.

7. Assists families in facilitating purchases of required items or household accommodations to meet Level 1 licensing standards.

8. Supports children living with relative caregivers to remain outside of foster care.

9. Assists relative and non-relative caregivers to complete required training to meet the Level 1 licensing requirements.

10. Completes Level 1 licensing documents with families and coordinates with CPA staff, as needed.

11. Coordinates and communicates status and services with Kinship staff delivering Navigation services to the families.

12. Demonstrates leadership role as necessary or indicated (i.e. identifying challenges or issues within the program; participation on project and / or team / peer committees as directed;

acts as a positive representative for the program)

13. Meets with Program Manager on a weekly basis for supervision to review work with families, training and professional development and program development.

14. Works within the philosophy and mission of the Children’s Home Network.

15. Participate in quality assurance reviews of the program services, outcomes and records.

16. Other related duties as assigned.

MINIMUM QUALIFICATION REQUIREMENTS :

Language : Spanish and Creole (PREFERRED)

Education & Experience :

Bachelor’s Degree from an accredited university or college in Human Services or related field, with one (1) year of experience working with children and families. Bi-lingual preferred.

Licenses & Certifications :

  • Must possess and maintain a valid Florida driver’s license with no record of criminal driving offense of license suspension.
  • Must possess means of providing job related transportation and show proof of required insurance. Must be insurable under CHN’s current auto insurance policy.
  • Must be able to work flexible hours, including evenings and weekends.
  • Must successfully complete and maintain FDLE clearance, Federal background and state criminal background check and sexual predator screening.
  • Must possess Child Welfare Certification approved by the Florida Department of Children and Families.

Knowledge, Skills and Abilities :

  • Ability to communicate effectively, verbally and in writing.
  • Ability to establish effective working relationships with family’s, coworkers, supervisors, and other individuals.
  • Possess effective interpersonal skills. Knowledge of agency’s organizational structure, standard operating procedures, and policies.
  • Knowledge of child abuse or neglect reporting procedures and methods.
  • Knowledge of quality documentation as required by agency standards, rules, and regulations.
  • Ability to work as a multi-disciplinary team member in a positive productive manner.
  • Knowledge of agency's organizational structure, standard operating procedures, and policies.

ESSENTIAL PHYSICAL SKILLS :

Visual : ability to safely operate a motor vehicle, read printed materials, and to make observations of physical facilities.

Hearing : ability to understand and comprehend spoken dialogue in individual and group settings (with or without hearing device).

Ability to communicate clearly and effectively.

ENVIRONMENTAL CONDITIONS :

Work indoors in an office environment.

Works in the community and conducts home visits

High level of emotional discussions, requiring self-awareness and understanding of professional boundaries.

SUPERVISION RECEIVED :

Weekly individual and team supervision during which time training is provided, individual families concerns are addressed, and individual areas of professional growth are reviewed.
