Position Summary
The PACT Team Clinical Coordinator (LPC, LCSW, LMFT) LMHP in conjunction with the PACT Program Manager leads a multidisciplinary team serving those with severe mental illness.
The PACT Team Coordinator duties include providing leadership, assessing, developing, coordinating, and providing treatment and rehabilitation and support services.
directs the day-today clinical operations of the PACT team including manage a team of clinicians, program budgeting, reviewing documentation, providing licensed clinical supervision to appropriate staff.
Required Education and Experience
Master’s Degree from a program that prepares individuals to practice counseling (60-hr Master’s Program), which is offered by a college or university accredited by a regional accrediting agency.

Two years of post-licensure clinical experience and one year of supervisory experience, preferably in a setting working with persons with severe mental illness.
Prefer Licensed Professional Counselor (LPC), Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), Licensed Substance Abuse Treatment Practitioner (LSATP), Certified Psychiatric Clinical Nurse Specialist (CPCNS).
Related Knowledge, Skills and Abilities
Knowledge of evidenced based practices to include but not limited to; Cognitive Behavioral Therapy (CBT); emergency interventions;
family, group, and individual therapies;, psychosocial education; risk assessments; sex offender treatment; substance abuse interventions; etc.
Ability to supervise employees, providing direct training when needed; Knowledge and ability to provide clinical supervision to QMHP-Es, Interns, and / or LMHP-Es
Knowledge of community mental health and public health in order to provide advocacy and enhance an individual’s treatment and access to community resources.
Knowledge of psychiatric / medical medications / treatments.
Ability to drive and must possess a valid driver’s license. May be called upon to travel to any Horizon worksite; transport clients, provide services to client @other locations.
Regular attendance at trainings and Horizon meetings may require travel to a variety of Horizon sites; Ability to participate in and successfully pass behavioral intervention training;
includes both verbal de-escalation skills and physical skills techniques to be utilized with consumers as needed.
