HOME HEALTH PHYSICAL THERAPIST (RPT) We have a great opportunity for a PRN or Part-Time Home Health Physical Therapist (RPT) in Muncie, Alexandria, and surrounding areas.

Responsible for educating patients and caregivers, while preparing the patient / caregiver for discharge as appropriate.
Job Responsibilities Provide skilled physical therapy in accordance with the physician’s plan of treatment. Observe, record, and report the client’s reaction to treatment and any changes in the client’s condition to the physician, DON and Clinical Manager, according to policy.
Develop, implement and revise the client’s plan of care. Promote coordination of care through interdisciplinary case conferences and communication with physician and field staff.
Participate in the agency’s quality improvement program. Perform therapy interventions utilizing standard physical therapy techniques and skills as appropriate for the condition of the patient.
Education, Licensure(s) and Qualifications . Minimum of 2 year experience as a Physical Therapist required. Previous Home Health experience is preferred.
