A company is looking for a Care Coordinator - Clinical Operations.Key Responsibilities : Provide outreach services via phone and other engagement channels to complete a Social Determinants of Health (SDOH) screening assessmentMake referrals to the client's in-house clinical social work team based on SDOH assessment resultsDocument all inbound and outbound inquiries, take appropriate actions to resolve them, and maintain a professional demeanor when interacting with customersRequired Qualifications : High School Diploma or equivalent1 plus years of experience in accessing community resources to help customers with basic needsAbility to adapt to change and problem solveKnowledge of managed care preferredBi-lingual Spanish and English language proficiency required
