Job Description
What you will be doing :
- Perform wellness exams and assessments for participantsat their place of residence in designated clinics or viatelehealth.
- Assess new patients by gatheringinformation reviewing diagnoses and analyzing medical testresults.
- Develop complete plans of care toaddress the needs of patients.
- Educatepatients about their care plan and health care options and assistthem in making key decisions.
- Revise careplans based on patients’ progress and input fromphysicians and other medical providers.
- Updatepatient medical records after each encounter.
- Offer emotional support for patients and theirfamilies.
- Obtain and record patients medicalhistory and symptoms including medicine reconciliation.
- Conduct HCC coding verification and riskadjustment.
- Collaborate with Primary Careproviders and case managers / team to ensure the completion ofholistic patient care.
- Complete patient chartswithin 72 hours of patients’ visits.
- Abide by all HIPAA rules and regulations and maintainpatients’ confidence by completing and safeguardingmedical records.
Experience youwill need :
- Current California NursePractitioner license.
- Extensive knowledge ofmedicine and anatomy.
- Current DEA licensure / Prescriptive Authority posthire.
- ActiveCPR / BLS Certification.
- Bilingual required English and Spanish.
- NursePractitioner certification from either the ANCC or AANP.
- 2 years of clinical experience as an NP in either PrimaryCare Family Medicine Internal Medicine Gerontology or PalliativeCare.
- Home care experience is aplus.
- Working knowledge of chronic caremanagement and various medication options.
