Job Details :
- Employed outpatient opportunity
- Board-certified Internal Medicine, Family Medicine, or Geriatrics
- 50% clinical 50% admin
- clinic hours are 7 : 30 AM 5 PM, M F
- The SMD demonstrates accountability for outcomes, strong clinical care, and cost-effectiveness for each patient in their panel of at least 200-225 risk adjusted patients
- Act as a health coach, rather than just a consultant for sickness, by helping patients set short and long-term health goals, partner with the patient to work toward the goals, and frequently follow up on those goals on the path to improved health for their patients
- Manage day-to-day clinical and business operations to improve the clinical and financial performance of the center, as the true dyad partner to the Center Manager / Center General Manager
