Position Purpose The START Coordinator provides person-centered, community-based clinical care coordination services for the participants in the START program.

The team provides community-based crisis intervention for individuals with IDD and mental health needs. The START Coordinator will develop the evaluation and intervention plans for the individuals served.

Essential Duties Maintain an active caseload of individuals referred for START Services Must successfully participate in and complete the START Coordinator certification training within 12 months of employment.

This includes the demonstration of competencies in all areas associated with START coordination Understand how to use ecomaps as part of systemic analysis and provide consultation to systems / teams as needed using best practices Assist with referrals for START services as needed including screening for enrollment in START Ensure the coordination of support meetings and crisis plan development for individuals enrolled in START Provide outreach visits to the individuals home, day and vocational settings regularly Use the START Action Plan for case planning and strategizing Develop Comprehensive Service Evaluations, Cross-Systems Crisis Prevention and Intervention Plans, intake / assessments, admission / discharge summaries for resource center admissions or inpatient hospital admissions and any other applicable documentation of services provided Develop materials and present cases at Clinical Education Team Meetings as assigned Complete all required data entry expectations Provide on-call support on a rotational basis as part of a START team.

Participate in the Center for START Services National Network, which brings together projects from across North America through an online resource center, conference calls, select study groups and other meetings and trainings Knowledge, Skills and Abilities Knowledge of IDD and behavioral health needs, and ability to interact effectively with individuals experiencing those disorders Knowledge of interdisciplinary teams, and ability to communicate effectively with psychiatrists and other behavioral health clinicians Ability to promote the mission, vision, and values of RMHS in the workplace and community Demonstrated time management and organizational skills Excellent communication and meeting facilitation skills Ability to independently assess crisis situations and connect individuals to appropriate resources Knowledge of community resources related to behavioral health and IDD Comfortable utilizing de-escalation strategies as needed Knowledge of and compliance with all state and federal laws and regulations as they pertain to position, including HIPAA, scope of practice, documentation standards, ethics, etc.

Maintain appropriate boundaries and professional demeanor Cultural humility and responsiveness Minimum Qualifications Masters degree in social work, psychology, counseling, applied behavior analysis or other human service fields 2 years' experience working with people with IDD and behavioral health needs and their families The ability to work with complex systems and an interest in the population served Independent and organized with strong verbal and written communication skills Ability to complete START Coordinator certification within 12 months following employment Preferred Qualifications Bilingual in Spanish and English Essential Functions Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

Actively communicate with clients, caregivers, families, stakeholders, advocates and providers. Works with clients, families and appropriate community agencies as necessary to facilitate services for staff, stakeholders or clients.

Ability to meet / work with staff, stakeholders or clients in a variety of settings. Attends staff, team and department meetings.

Attends in-services, staffing and other meetings with supervisors approval. May be appointed to committees. Participates in agency and community planning and education.

Develops and maintains records, plans and reports. Lift and / or carry 20 lbs. Sit, stand and walk for reasonable periods of time.

Maintains prompt and regular attendance. Performs related work as assigned. Ability to drive personal or company vehicle Driving Requirements Valid drivers license Proof of motor vehicle insurance Personal vehicle in good operating condition for use during work No major violations in the past three years.
