Description :

The Days Services Coordinator assists individuals with disabilities to participate fully in meaningful activities in their communities.

The Day Services Coordinator may work with individuals on a one-to-one basis or in small groups, and serves as New Avenues point of contact for one or more community partners.

In addition to providing services and supports to assist in achieving individualized goals and outcomes, the Day Services Coordinator is responsible for documenting service provision in a timely manner.

This position will also be responsible for maintaining regular communication with each person served as well as team members regarding progress, outcomes, and unusual incident reports.

Responsible to promptly report to supervisor any circumstances that negatively affect the health, safety, or well-being of residents, staff, the agency or its equipment.

Responsibilities :

  • Accompanies individuals to community sites to provide Adult Day Supports, Vocational Habilitation, Individual Employment Supports or other inclusion services as specified by the I.P.
  • Assists individuals to identify personal goals and desired outcomes related to community inclusion, self advocacy, and / or employment.
  • Provides continuous positive role modeling for appropriate public behavior, positive attitude and interaction with community members.

Wears appropriate attire for community site.

  • Maintains progress notes and reports on participants’ outcomes. Collects and completes all necessary documentation and reports related to service provision and submits timely.
  • Transports individuals to meaningful opportunities in the community based on interest.
  • Establishes new community sites / relationships and serves as the agency point of contact for several community partners.
  • Reports all Unusual and Major Unusual incidents reports according to agency policy / procedure. Participates in investigations as required.
  • Participates in all regulatory and compliance reviews, as required. Ensure compliance with outside standards and regulatory agencies, including, but not limited to : DODD, OOD, ODJFS, OSHA, DOL, HIPAA, ODH, etc.
  • Ensures a positive relationship with all other employees in a manner that enhances the programs and services of New Avenues.
  • Exhibit a positive, respectful, and professional behavior toward individuals served, families, and New Avenues personnel.
  • This position is expected to meet the demands and needs of the people we serve by being flexible and adaptable to changing schedules.

This position’s schedule may include a variety of first shift, second shift, and some weekends.

Follow New Avenues policies and procedures as well as regulatory standards.

  • Establish three new partnerships within first year of service.
  • Ensures that all necessary documentation is completed within 24 hours of service delivery.
  • Ensures outcomes are tracked appropriately and provide information on services.
  • Ensures that schedule and mileage in Data Plus is entered in a timely manner and is meeting the needs of the individuals served.
  • Ensures that person centered services are completed by using Charting the Life Course model.
  • Will pursue learning opportunities to enhance professional development yearly.

Qualifications :

  • Two years of experience in DD field.
  • Must be at least 18 yrs. of age.
  • High School Diploma or GED.
  • Experience in day service and / or employment settings.
  • High energy level with enthusiasm for connecting to the community.
  • Must obtain and maintain CPR / FA, PBS, and Medication Administration certifications.
  • Demonstrated computer skills including MS Office and G-Suite.
  • Valid Ohio Driver’s License that meet agency standards.
  • Must have reliable transportation to transport individuals as needed.
  • Maintain auto insurance.

Physical Requirements :

  • Able to sit, stand, and walk at full capacity for at least eight hours and requires frequent bending, squatting, reaching above the shoulder, kneeling, balancing, pushing, and pulling.
  • Lifting requirements of this position are frequent lifting of up to 20 lbs & occasional lifting of up to 50 lbs.
  • This position requires employees to use both hands for simple grasping, firm grasping & fine manipulations.
  • This position requires exposure to environmental factors such as possible exposure to COVID, aggressive client behavior, over-the-counter chemicals & products, prescription medications, etc.
  • This position requires that no treatment and / or medication adversely affect this person's ability to work.
