Behavioral Health Care Manager
Pride Health is hiring an outpatient Behavioral Health Care Manager for our client in Washoe County Nevada. This is a hybrid position with some travel.
Mileage reimbursement is offered.
Schedule : 8 am-5 pm Mon-Fri
Pay : $30-$46 hourly
Contract : July 15th-October 12th (Contract to hire)
Responsibilities :
- Will need to go to members’ homes and the community in Washoe County NV. Will work remotely otherwise.
- Coordination of member’s behavioral health care through the health care continuum.
- Collaborate with member, family, provider, facility, and health plan care team (to include PCP, COC, Medical Directors, and Community Services) to develop and implement an ongoing plan of care to obtain best outcome
- Provide health education, coaching, and treatment decision support for members
- Meets with members in their homes, at their providers’ offices, or inpatient facilities, if requested, deemed, appropriate, or warranted located in Clark or Washoe County.
- Ensure the survey is always ready for all lines of business according to the National Committee for Quality Assurance (NCQA) and state regulations for case management services.
- Perform other duties as assigned.
Requirements :
- 3 years of experience in a behavioral health setting.
- One year of Case Management is preferred.
- Active License in Nevada, licenses accepted : LMFT, CPC, and LCSW.
- CCM certification preferred.
- Active Drivers license
