This position will cover Southern Fairfield County
The Hospice Clinical Manager is responsible for ensuring that exceptional, quality, person centered care is provided by the Hospice Nursing and Coordinator Teams.
This role is responsible for creating a positive culture of accountability by the team. This position will be responsible for daily clinical operations, including insuring HCHB workflow is completed on a timely basis so patient care is not delayed, supervision of nursing clinicians in the field as well as hospice and volunteer coordinators to provide guidance and monitor performance of team;
providing necessary training and performance feedback.
The Hospice Manager of clinical services shall have primary authority and responsibility for maintaining the quality of clinical services.
Responsibilities :
Perform clinical workflow process tasks in EMR (HomeCare HomeBase) in conjunction with other clinical management and clinicians to ensure timely completion of orders so patient care can be completed quickly and within any necessary regulatory, payor, or other timelines.
Responsible for day to day clinical operations of hospice nursing and coordinator team
Coordination and management of services rendered to patients and families by team members under his / her supervision
Works closely with the Manager of Support Services to ensure quality of clinical services for hospice patients
Provide feedback to clinicians on documentation and best practices.
Provide leadership to clinicians in the field to help them improve their effectiveness, and symptom management skills
Leads Inter-disciplinary conference to ensure care is appropriate and within regulatory compliance for Hospice Teams
Participate in Administrator on call rotation
Performs Joint visits with nursing team per Connecticut Regulatory timeframes
Perform other clinical operation tasks or projects as assigned by the CCO
Management
Responsible for supervision of RN, LPN and Coordinator team members.
Supervision of assigned nursing personnel in the delivery of nursing services to patients and families.
Direct evaluation of the clinical competence of assigned nursing personnel and participation with Manager of Support Services in evaluation of other direct service team members
In conjunction with CCO maintain staffing levels aligned with business needs.
Evaluate potential hires or transfers to assure that candidates meet both technical requirements and the behavioral expectations and cultural fit with RVNAHEALTH.
In conjunction with QAPI, establish / update training and professional development plans which include department metrics.
Evaluate performance of clinical team against goals, action plans and outcome metrics (both technical and behavioral) and address performance improvement needs.
Assist with the proper orientation, training and competence of team on an ongoing basis.
Assist clinicians and coordinators meet established metrics.
Evaluate employee performance throughout the probationary period and on a regularly scheduled basis thereafter.
Participation in Policy and Procedure reviews that affect hospice clinical services
Lead and model appropriate adherence to all internal policies, procedures, practices, laws, licensure and regulatory requirements.
Ensure that all support staff adhere to the same.
In conjunction with the CCO, participates in program planning, budgeting and evaluation activities as related to hospice clinical services.
Organize, schedule and distribute work among assigned staff.
Keep staff informed of pertinent policies and procedures affecting the department and / or their jobs; create an atmosphere in which upward communication from employees is encouraged.
Administer human resources policies and procedures as established by agency policy
Qualifications :
Master of Science in Nursing from an accredited program in Nursing with a minimum of one year fulltime clinical experience in hospice or;
A Baccalaureate degree in nursing and a minimum of three years of fulltime clinical experience with at least one year in hospice
Current license to practice as a Registered Nurse Connecticut.
