Position Summary Educator will create personalized patient education plans (on diabetic, metabolic conditions and nutrition, etc), focusing on steps that aim to prevent complications, and work to manage / stabilize diabetes and other disease.
Educator will routinely collaborate with patients, family members, and health care team members for adjustments in patient plan of care, as required.
Under the general direction of Endocrinologist, Diabetic Educator will oversee the planning, implementation, and evaluation of education services for Diabetes management and prevention for the Endocrinology / Metabolic Medicine program.
Educator will ensure that high quality DSMES is delivered through an organized processes. Aside physician team, they will develop structure for practice's educational program, designing patient curriculum, and coordinating educational tools.
They will implement best practices in education, and assist patients in meal planning. They will also educate and teach patient to operate insulin pumps, CGMs, related applications, etc.
Educator will suggest treatment changes to Endocrinologist as needed as appropriate per internal protocols. They will serve as an Endocrinology and nutrition liaison within the physician network, and will actively participate in educations / clinical protocol development, the overall patient assessment, the referral process, education and quality related projects, and represent the programs mission and values.
They will be responsible for the educational program contents, teaching aids, materials, and logistical planning for such.
Clinician will report to the practice administrator, and clinically, will seek clinical input from MGM Endocrinologist (s).
Role may be asked to assist and partake in additional clinic duties, as needed, within the practice. See core duties for more responsibilities.

- Experience 5 - 7 Years Patient Clinical Education Experience - Required 3 - 5 Years Clinical Ambulatory Practice Experience - Required 1 - 2 Years Formal Nutrition and / or Diabetic Education Experience - Preferred
- License & Certification Professional Certification - Required CDE (Diabetic Educator Certification) required; Licensed Nutritionist required
- Core Job Functions Identifies patient education needs and may assist in patient teaching as directed by the provider for appropriate health maintenance and management of illness.
Communicates effectively with patients, their families and other members of the health care team. Host individual counseling sessions and / or classroom style group sessions as appropriate- at a variety of SJC sites.
Educate patients in monitoring blood glucose for the purpose of medication control and adjustment for both insulin and noninsulin-dependent diabetics, as well as diet education, teach routine diabetic care (foot care, eye care, lab schedules, etc), promote healthy lifestyle, proper monitoring techniques, educate on disease progression, and proper home care for patients and caretakers alike.
Provide in-depth nutritional care plans and tools. Refer patient to other programs / community programs, as needed. Assist in other Endocrinology clinical practice areas and initiatives, as needed, per management request Assist in development of policy, clinical protocol and procedures surrounding diabetic and nutritional education in the practices in an effort to improve quality, increase referrals, and maximize patient engagement and attendance.
Ensure there is an annual ongoing continuous quality improvement project each year surrounding the program. Analyze patient data and outcomes semi-annually for management to show effectiveness of program / referrals.
Represent Metabolic medicine program and mission in the local community and across the health system. Develop collaborative relationships with other departments, quality, service lines, and community health care agencies to facilitate and support quality diabetes care.
Periodically work with clinical staff and host in-services regarding diabetic diet best practices. Manage and track incoming referrals for education.
Collaborate with billing and coding to streamline billing process for such services. Correspond with referring providers and teams as warranted per patient condition and plan adherence.
The Nutrition Care Progress is used to determine a nutrition diagnosis and develop interventions using evidence-based research.
Nutrition Care is documented appropriately in the electronic medical record.
