Here is what is needed to be part of this amazing team :
- Bachelor’s degree required . -Master’s degree strongly preferred.
- Minimum three (2) years’ experience as a social worker in a healthcare related setting.
- Long term care and / or rehab experience preferred
- Knowledge of the rules, regulations, and guidelines that govern nursing care facilities.
- Excellent Communication skills ability to communicate with personnel, residents, family members, visitors, government agencies / personnel and the general public.
- COVID vaccination is required.
Responsibilities :
- Pre- Admission & Discharge Planning
- Psycho-social assessments of residents
- Work with interdisciplinary team members to create & implement care plan for residents
- Provide support & resources for residents and residents families to achieve optimal care of residents
- Maintain resident confidentiality.
- Advocate for residents and act as a liaison between residents and resources in the community
- Ensure residents social & emotional needs are met
- Promote resident independence & positive self image

Pay in Lieu of Benefits : extra $1.50
Waiving medical only : extra $1.50 per hour
