Job Description
Summary
Responsible for the timely coordination of the referral process including the review / response / coordination of care for patients referred to Graham Hospice for evaluation / admission and / or transfer process of patients to the Hospice agency.
As a liaison between the Graham sales and operational teams, the intake coordinator will utilize strong communication and organizational skills to increase admissions by coordinating efficient and quality patient care.
Essential Duties and Responsibilities
include the following. Other duties may be assigned as necessary.
- Serves as the first line of customer service for internal and external customers.
- Receives and documents patient referrals to hospice services, ensuring compliance with applicable coverage criteria.
- Reviews referral for appropriate care setting and communicates with care center, Business Development, and referral source when appropriate.
- Gathers appropriate information to assist the clinical team in scheduling the evaluation visit.
- Communicates pertinent patient information to the care center team via approved processes.
- Coordinates with other departments for insurance verification and payer authorization as needed.
- Professionally communicates and follows up with referral sources regarding required documentation for a complete referral.
- Communicates and follows up with Business Development regarding pending admissions as needed or as directed by Intake Team Lead.
- Meets productivity expectations with quality documentation of complete referrals including all required follow up.
- Coordinates the evaluation / admission process.
- Complies with policies, procedures and regulatory mandates including but not limited to abiding to the terms of the Graham Compliance Program.
- Performs other duties as assigned.
Qualification Requirements

- High School diploma or GED equivalent.
- Two (2+) years of administrative experience in a healthcare environment.
- Reliable internet service and ability to office from home with minimal disruption to the workday.
- Must be familiar with general use and functions of the computer, such as, usernames and password concepts; internet; e-mail;
- navigation of computer desktop, including starting programs, using files, and windows, effectively use navigation buttons and tool bars;
ability to self-manage online HR services and online training programs.
- Home health or Hospice intake experience, preferred.
- Homecare Homebase experience, preferred.
- Ability to identify patient needs and connect them to the appropriate resource.
- Knowledge of Medicare / insurance reimbursement issues and regulations to include coverage criteria.
- Demonstrated organizational skills within a complex, fast-paced environment.
- Completes work with accuracy and within assigned time frames.
- Requires minimal supervision and is self-directed.
- Adheres to company infection control and safety policies, including education, reporting, and practice implementation specific to job position.
- Develops cooperation and collaborative work efforts that generally benefit all involved parties.
- Demonstrates the initiative to meet the needs of the organization by assisting co-workers when workload permits.
- Adheres to policies and procedures and is flexible to changes in duties and responsibilities.
- Possesses strong Microsoft Office skills.
- Excellent oral / written communication, presentation, critical thinking, and interpersonal skills
