Central City Concern is an innovative nonprofit agency providing comprehensive services to single adults and families in the Portland metro area who are impacted by homelessness, poverty and addictions.

Letty Owings Center (LOC) has helped young mothers to become sober, healthy and hopeful about the future. Recovery is more complex during pregnancy and while raising young children, we offer a safe, inclusive environment for women to live with their children while in treatment for substance use disorders.

Our residents often are able to stay with their children, who might otherwise be placed in foster care.

This position provides case management to pregnant and / or parenting women in a residential setting; by providing family court advocacy, accessing medical care, attending family decision meetings with DHS Child Welfare, assisting in accessing recovery-community supports, and obtaining necessary resources for independent living.

This position also supports clients’ transition from residential treatment, to outpatient, and into housing. The LOC Case Manager will be familiar with, and able to access resources on behalf of clients and will be a strong advocate for children and families.

This position is skilled in maintaining professional boundaries and providing direct services through a trauma-informed lens.

This position has access to mental health billing and service records.

Schedule : Monday - Friday, 8 : 00am to 4 : 30pm

Wage : starting at $22.46 per hour

MINIMUM QUALIFICATIONS :

  • High school diploma, GED or demonstrated ability to read and write at a sufficient level.
  • Must possess a current driver’s license and qualify as an Acceptable Driver as designated in Central City Concern’s Fleet Safety policy.

Must pass an initial driver training within 60 days of being an approved driver and continued recertification training. Must maintain vehicle insurance coverage of a minimum of $100,000 / $300,000 personal auto liability coverage if using personal vehicle for CCC business.

  • Have or able to obtain Adult and Child / Infant CPR Certification prior to start date.
  • Must meet CCC privileging requirements as required by FTCA.
  • Experience working with families involved with child welfare preferred.
  • Must have basic knowledge of substance use disorders.
  • Ability to adhere to Central City Concern’s drug-free workplace which encourages a safe, healthy and productive work environment and strictly complies with the Drug-Free Workplace Act of 1988.

An employee shall not, in the workplace, unlawfully manufacture, distribute, dispense, possess or use a controlled substance or alcohol.

  • Will be required to carry a cell phone for work use. Cell phone to be provided by Central City Concern.
  • Must pass a pre-employment drug screen, TB test, and background check. This includes clearance by the DHS Background Check Unit.

Must be approved by the Childcare Background Check Unit.

  • The OAR prohibits A&D program staff from having within the previous 3 years a DUI conviction or diversion, a felony conviction, or a conviction under the Uniform Controlled Substances Act.
  • Prior to employment, must be approved by the State of Oregon Employment Department Child Care Division. Must complete consent for criminal records identification check pursuant to ORS 181.

537. This program receives funds from the Federal Child Care Development Fund block grant via a contract with the Child Care Division for Alcohol & Drug Abuse Programs.

  • Physical ability to bend, stoop, kneel, squat, twist, reach, pull and lift heavy objects.
  • Must be able to climb stairs several times a day.
  • Willingness and ability to work flexible hours, including some weekend and evening hours.
  • Must adhere to agency’s non-discrimination policies.

MINIMUM QUALIFICATIONS : PSS OR CRM

Must be registered with MHACBO as a Certified Recovery Mentor OR Certified Peer Support Specialist (or Peer Wellness Specialist) and be registered as a Traditional Health Worker on the THW registry.

MINIMUM QUALIFICATIONS : QMHA-R or QMHA-I

Must be registered with MHACBO AND Relevant Bachelor’s Degree or 3 years relevant work experience towards QMHA.

MINIMUM QUALIFICATIONS : CADC-R or CADC-I

CADC-R or CADC-I must hold an active credential with MHACBO.

ESSENTIAL DUTIES AND RESPONSIBILITIES :

  • Provide case management services for women with young children residing at LOC for residential treatment services and Outpatient services in CCC.
  • Develop, implement, and maintain a case plan to provide all wraparound services as needed for clients.
  • Develop and maintain a positive working relationship with all parties involved with clients’ cases including, but not limited to, DHS caseworkers, DHS worker assigned to transport and monitor child visits, VOA outreach workers, defense attorneys for all family members, physicians and mental health providers working with the family, probation officer and other community partners.
  • May facilitate FIT (Family Involvement Team) specific tasks at the direction of the clinical supervisor.
  • Prepare case notes that clearly document all services provided for clients and maintain charts and records in a legible and accessible form.
  • Assist client in obtaining Primary Care Physician (PCP), if necessary.
  • Assist clients in any needed medical follow-up and transport them to their medical appointments.
  • Attend regular clinical meetings and provide current information to team regarding client’s participation in program and other activities.
  • Transport clients and their children to appointments, hearings, support groups, and other services as needed.
  • Attend all client’s court hearings and family unity meetings.
  • Assist clients in applying for eligible benefits, including Oregon Health Plan, food stamps, cash assistance, housing assistance, child care assistance, job training programs, etc.
  • Work closely with LOC primary counselors in case planning; facilitating referrals to after care services, including housing, vocational rehabilitation, outpatient A&D treatment, mental health services, etc.
  • Participate in compliance, quality assurance and quality improvement activities as directed.
  • Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security.

Support compliance with all privacy and security requirements pursuant to community partners’ and outside providers’ patient confidentiality agreements, including privacy and security requirements for EMR access.

This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee.

  • Attend all mandatory CCC trainings in a timely manner.
  • Perform other duties as assigned.

OTHER DUTIES IF CADC-R or CADC-I :

Cover SUDS related groups in the event the regular facilitator is unavailable and document activity in EPIC.

OTHER DUTIES IF QMHA-R or QMHA-I :

Cover Peer led groups, Skills Training and Development groups, or provide some limited Psychoeducational service, in the event that the regular facilitator is unavailable, and document that activity in Epic.

SKILLS AND ABILITIES :

  • Ability to work effectively in a residential and Outpatient treatment environment with mothers and their children, clinical and program staff, and other members of a team.
  • Ability to effectively communicate with clients and children; coworkers; corrections and court personnel, child welfare workers, and other community referents.
  • Understanding of, and ability to work with the family court and juvenile justice system.
  • Ability to articulate treatment program’s goals and position in a courtroom setting.
  • Ability to serve as a positive role model for families striving to engage in recovery.
  • Ability to understand and support family re-unification or termination requirements, and self-sufficiency plans developed by other agencies.
  • Ability to manage complex and multiple tasks and prioritize appropriately.
  • Understanding of HIPAA, 42 CFR, confidentiality issues, personal and professional boundaries, and a demonstrated ability to incorporate them into daily job performance.
  • Comfortable working with people from a diverse range of ethnic and socio-economic backgrounds, including those with dual diagnoses and / or other mental or physical disabilities.
  • Ability to foster positive relationships with community partners.
  • Knowledge of and ability to utilize de-escalation techniques when encountering hostile individuals or volatile situations.
  • Understanding of substance use disorder as a disease, including treatment and intervention methods.
  • Ability to work in an environment with a high degree of ambiguity and change.
  • Ability to manage time and meet deadlines.
